Application Notice for Credit Transfer (Fall Semester 2025)
一、申請對象 Applicants
114年09月入學新生（含秋季班外籍生、港澳僑生）。
Newly-admitted students in Fall Semester (including International & Overseas Chinese students).
Transfer students and master’s students admitted directly to the Ph.D. program are required only to submit an application for course recognition.
二、申請條件 Eligibility

四、申請程序 Procedures
須同步完成「線上抵免系統」登錄及「紙本作業」申請。
Both the online system and paper-based application are required.
■ 紙本作業 (Hard Copy)
請備齊以下文件並自行預留送審時間：
Prepare following documents and reserve time for review:

- [English Version Download] - 歷年成績單正本：擬抵免科目須用螢光筆畫記。
Transcript (Original copy) - mark the transferred courses. - 課程大綱：擬抵免科目之教學大綱。
Syllabus / Course Outline.
■ 申請步驟 Application Steps
(1) 填寫學分承認表：
※內容不可塗改，若有錯誤請重新填寫。任何塗改處將不予受理。
Any corrections are NOT allowed. Please restart with a new form if needed.
(2) 原校系辦核章：
證明欲抵免課程「未列入原校畢業學分」。
Original school's department must stamp to confirm credits were not used for graduation.
(3) 送交學程辦公室：
經指導教授簽核後送件。請先送出線上申請，再繳交紙本文件至學程辦公室核章。
Submit to program office after advisor's signature. Online submission MUST be first.
※ 因有前置作業與審查流程，請務必提前備妥相關文件，自行送交原校及權責老師簽章完成後，再於申請期限內繳至學程辦理。
Please ensure all signatures are obtained before the deadline.
