Application Notice for Credit Transfer in fall semester 2025
一、申請對象：114年09月入學新生(含秋季班外籍生、港澳僑生)。
Applicants: Newly-admitted students in fall Semester (including International students、Oversea Chinese students)
※ 本校轉所生/碩士逕博生入學，採學程內學分承認申請即可
Transfer students and master’s students admitted directly to the Ph.D. program are required only to submit an application for elective courses to be recognized toward the program’s graduation requirements.
二、申請條件(Eligibility:)：參閱本學院研究生抵免學分規定

三、申請期限(Deadlines)：2025/08/01 ~ 2025/09/12。(According to Taiwan time)
四、申請程序(Procedures)：須同步使用"線上抵免系統"及"紙本作業"申請學分抵免
Both the online credit transfer system and paper-based application are required.
■紙本作業(hard copy)：
請務必提前(請自行預留送交權責老師審查時間)至本學程網頁/法規表單/下載抵免學分承認表，備齊以下文件：
Download “Application For Credit Transfer”(Kindly reserve adequate time for submission and review), and prepare following documents:
1. 學分承認表Application For Credit Transfer：

2.歷年成績單(正本)：擬抵免科目須用螢光筆畫記。

■線上抵免系統(credit transfer system)：
於申請期限內，上傳上述文件至抵免系統https://nckustory.ncku.edu.tw/credit/index.php辦理後續抵免事宜。

■申請步驟(Application Steps)：
(1).填寫本學院學分承認表：
Fill out the application form.
※若本表課程名稱之內容及學分證明聯之學分數經塗改將不予受理，請重新騰寫本表。
Any corrections to the course information, including the credit hours, are not allowed on this application. Please start over by filling out a new form for submission if you find any mistakes in the course information.
(2).請原就讀學校系辦於承認表蓋章，證明欲抵免課程學分未列入畢業學分
The department office of your original school must stamp the application form to confirm that the courses to be transfered were not included in your graduation credits.
(3).指導教授簽核後送至學程蓋主任章。
Once the advisor has signed, submit the document to the program office for official stamping.
※請先送出線上申請，再將紙本文件交至學程辦公室核章
The online application must be submitted before handing in the hard copy application form to the program office.
※因有前置作業，請同學務必提前備妥相關文件，自行送交原校及權責老師審查簽章完成後，再於申請期限內繳至學程辦理。
Please ensure that the documents are submitted to your original institution and the advisor for review and signature before submitting them to the program office within the application deadline.
