112學年第1學期抵免學分申請通知

Application Notice for Credit Transfer in fall semester 2023

一、申請對象：111年09月入學新生(含提前入學生、秋季班外籍生)。

Applicants: Newly-admitted students in Fall Semester (including early enrollment  students, foreign students)

二、申請條件(Eligibility:)：參閱本學院研究生抵免學分規定

三、申請期限(Deadlines)：2023/08/01 ~ 2023/09/08。

四、申請程序：請務必提前(請自行預留送交權責老師審查時間)至本學程網頁/法規表單/下載抵免學分承認表，備齊以下文件：

1. 學分承認表：

Application For Credit Transfer

(1).填寫本學院學分承認表：

Fill out the application form and ask a Advisor to sign.

(2).須請原就讀學校系辦蓋章證明

Students are required to ask the stamp from the department of original university.

(3).送至學程蓋主任章。

Submit to the program office to get a stamp.

(4).若本表課程名稱之內容及學分證明聯之學分數經塗改將不予受理，請重新騰寫本表。

Any corrections to the course information, including the credit hours, are not allowed on this application. Please start over by filling out a new form for submission if you find any mistakes in the course information.

 2.歷年成績單(正本)：擬抵免科目須用螢光筆畫記。

Transcript (mark the transferred courses)

3.課程大綱(擬抵免科目)。

Syllabus/ Course Outline

4.於申請期限內，上傳至抵免系統https://nckustory.ncku.edu.tw/credit/index.php辦理後續抵免事宜。

※因有前置作業，請同學務必提前備妥相關文件，自行送交原校及權責老師審查簽章完成後，再於申請期限內繳至學程辦理。
