Application Form for Elective Courses Counting Towards Graduation Credits – 115-1 2026 Fall Semester
The deadline for submitting the application for program approval is September 15, at noon.
Students who need to apply should have the form signed by their advisor in advance and attach the syllabus for each course. Please submit the documents to Room 44C10 (schedule an appointment in advance).
Once received, we will assist in obtaining the director's approval.
